Patient Info

Your First Visit: Naturopathic Medicine

Initial visits for naturopathic medicine are between 1 1/2 to 2 hours. Follow up visits are generally 45 minutes.

There are three things that are very helpful in understanding chronic or complex issues. If you are able to send this information beforehand it would contribute greatly to my ability to help you:

  1. Most recent labs tests if performed in the last year, plus any pertinent imaging or radiology reports
  2. A chronological time line of your life and medical history: Of interest are trauma, injuries, accidents, surgeries, illnesses, initial pain, diagnoses, stressful events, major life events, toxic exposures, etc. Put the date (or just the year) in the first column and the event in the second column in the order in which they happened.
  3. A list of each of your medications and supplements, brand names, the amount you are taking, and how long you have been taking it.

You may FAX this information to the office where you are scheduled or bring it with you to your first visit.

Your First Visit: Acupuncture

Initial acupuncture visits are scheduled for 1 1/4 to 1 1/2 hours. Follow up visits are generally 45 minutes to 1 hour. It is helpful if you wear loose clothing, that is, pants and sleeves that can be rolled up. Do have a good meal on the day of your appointment and make sure you have had some water to drink. It is best to have time to relax after the treatment.

Before your visit please take time to print and fill out the following forms. Please bring the completed forms to your appointment. Thank you!

Demographic Form (print and fill out)

Medical History Form (print and fill out)

Privacy Policy (print, read, sign)

Informed Consent (print, read, sign)

Records Release (necessary if labs or records are to be forwarded from another medical facility you may make out this form, FAX or deliver it to the originating facility, or bring it in with you to your appointment.)

Payment is expected at the time of the appointment. At this time I am able to take checks or cash but not credit cards. Please call for a current fee schedule.

I am currently considered an in-network specialist with Cigna. Coverage varies, depending upon the individual policy. Note that coverage varies for naturopathic medicine and acupuncture. It is necessary to check your individual policy in conjunction with your diagnosis to find out if you are covered. I am not billing insurance companies at this time, but I am happy to provide you with a superbill that you may, in turn, submit to your insurance for possible reimbursement.
